| All participants will be required to provide picture identification when registering. Please have identification ready to assist in the registration process. |

| CREDIT: Participants who attend the class will receive a certificate of proof of attendance. By completing the entire class you will earn 8 CE hours and you must |
| complete the CE form in your participant packets and return it to the instructor, who will forward it to the Society. It will take 4-6 weeks to receive your credits. |
| If you need to have your CE credits immediately, there will be a $25 rush fee or $40 for overnight rush. It must be given to the instructor with the |
| CE form.It is not necessary to take the exam to receive the credit. PER STATE REQUIREMENTS, NO C.E. ISSUED TO PARTICIPANTS ARRIVING LATE |
| AND PARTIAL CREDIT WILL NOT BE GIVEN. |

| CANCELLATION: Cancellations received within 10 days of the institute will incur a $50.00 non-transferable fee. “No shows” will not receive any credit or refund. |
