    All participants will be required to provide picture identification when registering.  Please have identification ready to assist in the registration process.
     
    CREDIT:  Participants who attend the class will receive a certificate of proof of attendance.  By completing the entire class you will earn 8 CE hours and you must 
    complete the CE form in your participant packets and return it to the instructor, who will forward it to the Society.  It will take 4-6 weeks to receive your credits.  
    If you need to have your CE credits immediately, there will be a $25 rush fee or $40 for overnight rush.  It must be given to the instructor with the 
    CE form.It is not necessary to take the exam to receive the credit. PER STATE REQUIREMENTS, NO C.E. ISSUED TO PARTICIPANTS ARRIVING LATE
     AND PARTIAL CREDIT WILL NOT BE GIVEN.
     
    CANCELLATION:  Cancellations received within 10 days of the institute will incur a $50.00 non-transferable fee.  “No shows” will not receive any credit or refund.
